Avive Virtual Care· formerly Cloud Clinic

New patients

Getting started is simple.

To see one of our psychiatrists by telehealth, you’ll need a referral from your GP or specialist. Here’s how to get from referral to your first appointment.

How to start

Three steps to your first appointment

1

Get a referral

Ask your GP for a referral to a psychiatrist. GP referrals must be dated within 12 months of your appointment (specialist referrals within 3 months) for Medicare rebates. We may also ask your GP to complete our GP physical health check form.

2

Complete our intake form

Our secure online intake form takes about 10 minutes. It collects your personal details, Medicare information and payment details. Nothing is charged until our team confirms your appointment.

3

Book your appointment

We match you with the right psychiatrist. Once we have your documents, you’ll be invited to book your first telehealth appointment online.

Have your referral?

Our intake form is run by our secure patient portal, Halaxy. Please make sure you have a valid referral before you start.
